Here's what Aetna needs, and what you'll pay, and how the new $200 copay models are going to play out.
The CVS Caremark standard, Advanced Control and Value formularies have recommended Wegovy as their preferred weight loss GLP-1, and members who were previously covered by a CVS Health Plan on Zepbound have been automatically moved to a CVS Caremark plan on July 1, 2025. This preference remained for Aetna's formulary updates in 2026, and it will help Aetna members have fewer barriers to using Wegovy compared to any other competing weight-loss injection.
Preference does not equal universal coverage. Employers still have the choice whether to offer weight-management drugs. No matter what the plan documents specify, there is always a formulary preference, however, and 2026 saw something new: Caremark gave employers the opportunity to add the category within their plan documents, with member copays as high as $200 per month, which would make coverage affordable for the first time for the plan sponsor, as Delaware's state plan publicly did.
The clinical criteria follow the FDA label. Adults qualify with a BMI of 30 or higher, or a BMI of 27 or higher with at least one weight-related condition such as hypertension, type 2 diabetes, dyslipidemia, or obstructive sleep apnea. Aetna typically asks for a documented baseline weight, participation in a reduced-calorie diet and increased physical activity program, and confirmation you're not combining Wegovy with another GLP-1.
Renewals hinge on results. Continuation requests generally require at least 5 percent weight loss from baseline during the initial authorization, so make sure every weigh-in lands in your chart. Adolescent coverage (ages 12 and up) exists on some plans under the pediatric indication with its own criteria.
Members with established cardiovascular disease have a second route. Wegovy's March 2024 FDA approval for cardiovascular risk reduction lets prescribers submit under a disease-treatment indication, which can matter on plans with weight-loss exclusions.
Costs vary according to plan design, with approval. The standard copay plans usually cost $25 to $100 per month for preferred brands. Plans with the new structure for weight management could cost as much as $200 a month, which may be beyond out-of-pocket limits on some plans. Plan members with a high deductible are responsible for paying the negotiated rate up to the point of a deductible.
There are two discounts in succession. Novo Nordisk's savings card will reduce eligible commercial copay by up to $100 per month (Delaware's plan calls for the card to be used in conjunction with a $200 copay). And if your plan doesn't even cover it, you can get Wegovy from NovoCare Pharmacy or CVS retail for roughly $499 per month cash, and/or the federal TrumpRx channel for around $350, as they are expanding into 2026.
When submitting, include the file in the following order: baseline BMI and weight history, condition documentation relating to weight issues, and 3 to 6 months of notes from the lifestyle program. Please send all documents electronically via the Caremark portal to your prescriber, and wait for that to be completed in 24 - 72 hours.
If you are refused, read the letter and find out why. Category exclusions will require the employer conversation or self-pay. Clinical denials are responsive to appeals: First, internal review within 30 days (72 hours expedited); then, independent external review. Appeals that are complete with documentation from the BMI and program are successful, but they are relatively common.
